FORM A — Consent when filling in a contact form

“I, as the personal data subject, in accordance with Federal Law No. 152-FZ dated July 27, 2006 On Personal Data, give my consent to AIVA LLC (TIN 5015015263, address: Meshchersky Boulevard 2, Zarechye urban settlement, Odintsovsky urban district, Moscow Region, Russia) to process the following personal data: surname, first name, email address, phone number, company/organization, and request text.

Purpose of processing: response to the request, follow-up communication, provision of information about the Controller's Services.

Processing methods: collection, recording, systematization, accumulation, storage, updating, use, transfer to authorized persons when necessary, deletion, destruction.

Personal data is processed using automation tools. All data is stored on servers located in the Russian Federation.

Retention period: 1 (one) year from the date the request is received.

The consent remains valid until withdrawn. I may withdraw this consent by sending a written request to hello@aiva.chat or by mail to the Controller's address.

This consent is given by ticking the checkbox next to this text.”

FORM B — Consent when registering and accepting the offer

“I, as the personal data subject, in accordance with Federal Law No. 152-FZ dated July 27, 2006 On Personal Data, give my consent to AIVA LLC (TIN 5015015263) to process the following personal data: surname, first name, email address, phone number, organization / sole proprietor name (for legal entities and sole proprietors), job title, and other data provided by me during registration and use of the AIVA Service.

Purposes of processing: registration and maintenance of the Account; granting access to the AIVA Service; sending service notifications; technical support; invoicing and payment processing; compliance with the laws of the Russian Federation.

Legal basis: performance of a contract (Public Offer) to which the personal data subject is a party (subparagraph 5, part 1, article 6 of Federal Law No. 152-FZ); consent of the subject (subparagraph 1, part 1, article 6 of Federal Law No. 152-FZ).

Processing methods: collection, recording, systematization, accumulation, storage, updating, extraction, use, transfer to persons involved in rendering the Services, anonymization, deletion, destruction. Processing is carried out using automation tools. All data is stored within the territory of the Russian Federation.

Retention period: the entire term of the Account plus 3 (three) years after its deletion.

I have read the Privacy Policy and the Public Offer and agree to their terms.

My consent to the processing of personal data is given voluntarily, knowingly, specifically, and in an informed manner. This consent may be withdrawn by sending a written request to hello@aiva.chat.

I confirm that I am 18 (eighteen) years of age or older.”

RIGHTS OF THE PERSONAL DATA SUBJECT

The personal data subject has the right at any time to:

- withdraw this consent by sending a written request to hello@aiva.chat or by mail to the Controller's address; - obtain information about the data being processed; - require correction, blocking, or destruction of the data; - challenge the Controller's actions before Roskomnadzor: https://pd.rkn.gov.ru; - apply to a court for protection of their rights.

Withdrawal of consent does not affect the lawfulness of processing carried out on the basis of consent before its withdrawal. Withdrawal of consent to the processing of data required for performance of the contract (Public Offer) results in termination of the Services.

The period for reviewing a withdrawal request is 10 (ten) business days from the date of receipt.
